The first thing you need to do is to request information and share your email address with us. We will send you a welcome package through email that includes important information and a health form to complete. After you submit your health form, our medical team will review your case. Once you are approved for bariatric surgery, we will email you your acceptance letter.
With the acceptance letter you are cleared to proceed to scheduling through us. Together we will work towards your desired surgery dates.
